I really don't think there is such a thing as a "perfect" first date. Mainly, because the two people usually don't know that much about each other. They may know each other casually, maybe from work or through a friend, but this is a completely different type of situation than they are usually in. Mistakes will be made during that first date.

Planning and carrying out a perfect date is not possible. If one should by chance happen, that's great, but it would be karma or fate or something. Each person is different, you can never be sure how someone will react to something you do or say. Perfection? No. And who needs perfection anyway? That's just too hard to maintain.

I agree with the prior posters; there really isn't such a thing as a perfect first date. Normally one if not both of the individuals is slightly nervous, because you don't know what the other person likes/dislikes. You don't want to say the wrong thing, so you think carefully about everything you say. The more dates you have with a person, the more at ease you can be, and then there can possibly be a perfect date.

I think perfect dates happen. I don't think they do if you plan one. Planning anything almost never works out as you expect, because we can't foresee every contingency. Shit happens just as often as not.
I've had lots of perfect dates when I was young and single. They were generally with someone I already knew well and it was just like an extension of our friendship. Generally though we liked being friends better than sweethearts and went back to that. Usually. I lost a couple good friends over taking that next step.

Has anyone else found that? That it is very difficult to go back to a friendship after being in a relationship, specially if it was an intimate one.

I agree with Bikerhen. Sometimes it is possible to go back to being just friends, but other times it is not. I think it depends on how long the relationship lasted, and how long you were friends before. I once tried this with a guy that I had been friends with for as long as I could remember. We went on several dates and discovered that we were better off as friends. At first things were weird, but eventually we were able to continue being friends. However, if we hadn't been friends for so long before that, I don't think it would have worked.
